


Services Provided
COMPANION/HOMEMAKER/HOME COORDINATING:
Provides non-medical care for people that need assistance or additional care in home. Services usually require a 2-4 hour minimum. Services can be tailored to meet individual needs that may include:
COMPANIONS:
- Conversation, socialization, walks, and reading
- Confirming medications taken
- Assist with personal hygiene (non to minimal hands-on care)
- Outings (i.e., lunch, bingo, family reunions, and travel, etc.) driving to appointments, shopping, errands
HOMEMAKERS:
- Household chores (i.e. light housecleaning, ironing, laundry, pet care. Etc.)
- Meal preparation, meal serving, grocery shopping
- Yard care
HOME COORDINATING:
- Stop in visits to check well being
- Oversee and coordinate outside caring services
- Supervise household books, expenses, write letters
PERSONAL CARE WORKER AND HOME HEALTH AIDE:
Provides services established by a nursing plan of care provided to maintain health or to facilitate treatment of a medical condition. They are supervised and report to a registered nurse. Services usually require a 2-3 hour minimum and are divided in to four groups. .
- Companion and homemaker services
- Activities of daily living (ADL): mobility, toileting, hygiene, grooming, dressing, exercising and nutrition.
- Bath services
- Medically orientated services: tube feeding, simple dressing change, complex care with positioning and transfer,
vital signs, and glocometer readings.
PERSONAL CARE WORKERS PROVIDE:
- Companion and homemaker services
- ADL services
- Bath services
HOME HEALTH AIDES PROVIDE:
- Companion and homemaker services
- ADL services
- Bath services
- Medically oriented tasks
SKILLED NURSING SERVICES:
Registered nurses and licensed practical nurses on staff 24 hours per day, 7 days per week. Nursing care is provided under the direction of a doctor.
Services include but are not limited to:
- Patient education
- Family instruction
- Medication supervision
- Injections and would care
- Heart and blood pressure monitoring
- Nutritional counseling
- Case management

INTRAVENOUS (IV) THERAPY:
Specially trained registered nurses work with pharmacists to coordinate IV medications prescribed at home such as:
- Antibiotics
- Supplemental nutrition
- Blood products
- Chemotherapy
- Pain management
